Description
Arising from an intercostal nerve near the parasternal line, the anterior cutaneous branch pierces the intercostal muscles and the deep fascia to enter the subcutaneous tissue of the anterior chest wall, then courses toward the breast where it arborizes around the areola and overlying skin. Medial to the mammary gland lobules, the nerve runs superficial to pectoralis major but deep to the adipose layer and suspensory connective tissue (Cooper ligaments), crossing between lactiferous ducts on its way to the nipple-areolar complex. Segmental vessels accompany the nerve in the superficial plane. Small branches radiate laterally and superiorly across the anterior breast. For teaching anterior breast innervation, this pathway is the one you want when discussing sensation and postoperative numbness, because the anterior cutaneous branches (T2 to T6, depending on level) are commonly interrupted during periareolar incisions, medial lumpectomy approaches, and subcutaneous mastectomy flap elevation. It also frames a real pain complaint: intercostal neuralgia or post-thoracotomy pain can refer to the anterior chest and into the breast, and irritation of these terminal cutaneous branches can be mistaken for primary breast pathology.
