Running in lateral profile through the lower back, the lumbar vertebrae (L1 to L5) stack superior to the sacrum, their vertebral bodies separated by intervertebral discs shown as pale blue fibrocartilage. Posterior to the vertebral bodies, the vertebral arches, spinous processes, and transverse processes project dorsally and laterally, creating the attachment field for deep paraspinal muscle fascicles rendered in red. Pale fibrous bands track along the anterior and posterior margins of the vertebral bodies consistent with the anterior longitudinal ligament and posterior longitudinal ligament, while additional short connective tissue spans between adjacent laminae and spinous processes, consistent with the ligamenta flava and interspinous and supraspinous ligaments. Segmental stability is the theme. A lateral lumbar emphasis is the cleanest way to teach what resists flexion and what controls shear: the anterior longitudinal ligament limits hyperextension at the anterior column, the posterior longitudinal ligament and ligamenta flava tighten with flexion, and the multifidus and other deep extensors provide active restraint across individual motion segments. This is the anatomy behind common mechanisms of low back pain, including disc herniation that most often tracks posterolaterally at L4 to L5 or L5 to S1 where the posterior longitudinal ligament is narrower, and it also frames why hypertrophy of the ligamentum flavum contributes to lumbar spinal stenosis. Short fibers, short spans, big consequences. Surgeons and radiologists think in this layer-cake.
