Description
Posterior thoracic wall anatomy is centered on the rib cage and thoracic vertebrae, with the subcostal muscles (mm. subcostales) highlighted on the internal surface of the lower posterior ribs. These thin slips arise near the angle of a rib and pass inferomedially to insert on the rib one or two levels below, spanning multiple intercostal spaces deep to the internal intercostal layer. Adjacent landmarks include the costovertebral joints, spinous processes of the thoracic vertebrae, and the superior margin of the pelvis inferiorly, giving clear orientation of superior to inferior thorax into the lumbar region. Subcostal muscles matter because they are easy to overlook in routine teaching of intercostal layers, yet their fiber direction and segmental attachments help explain subtle mechanics of forced expiration and stabilization of the posterior rib cage. Their deep position also places them near the intercostal neurovascular bundle running in the costal groove and near the parietal pleura, a relationship that becomes concrete when discussing posterior thoracostomy placement or intercostal nerve blocks.
