Description
Posterior thoracic wall anatomy is presented with the skin removed, exposing the ribs as they arc laterally from the thoracic vertebrae and the internal intercostal musculature occupying the intercostal spaces. Between adjacent ribs, the internal intercostal fibers course inferoposterior to inferoanterior (opposing the external intercostals), forming an interosseous layer deep to the superficial back musculature at the scapular level. The scapulae lie posterolaterally over ribs 2 through 7, with the proximal humerus and deltoid region framing the shoulder while the vertebral spinous processes define the midline. Fiber direction is clear. Internal intercostals matter because they are one of the primary expiratory muscles of the chest wall, drawing the ribs inferiorly during forced expiration and stabilizing the intercostal spaces during coughing. Posteriorly, near the rib angles where the internal intercostal layer transitions toward membranous coverage, the intercostal neurovascular bundle runs in the costal groove along the inferior border of each rib, typically between the internal and innermost intercostal layers, a relationship that drives needle placement for intercostal nerve block and explains why rib fractures often injure the intercostal vessels. This view also supports teaching safe surgical planes for posterolateral thoracotomy and chest tube passage by orienting the clinician to rib spacing and intercostal layer depth.
