Description
Supraacetabular groove sits on the lateral (gluteal) surface of the ilium, immediately superior to the acetabular rim and inferior to the flared ala (iliac wing). In a fixed lateral perspective of the hip bone, the furrow lies just cranial to the acetabulum and adjacent to the anterior acetabular region, where the ilium thickens as it approaches the anterior inferior iliac spine. Bony landmarks around it typically include the iliac crest superiorly, the acetabulum inferiorly, and the smooth gluteal surface extending posteriorly toward the greater sciatic notch. Small, but specific. Clinically, the supraacetabular groove matters because it marks the origin of the reflected head of rectus femoris, a detail that explains why proximal rectus femoris injuries can involve pain deep to the anterior hip and why avulsion patterns differ between the direct head at the anterior inferior iliac spine and the reflected head from the supraacetabular region. Orthopedic surgeons also reference this periacetabular area when discussing acetabular fractures, periarticular osteotomies, or screw trajectories in pelvic fixation, where a few millimeters of bony geography can separate stable purchase in the ilium from joint violation.
