Description
Positioned at the thoracolumbar junction, the T12 vertebra sits inferior to T11 and superior to L1, marking the transition from the kyphotic thoracic spine to the lordotic lumbar spine. The vertebral body lies anterior to the vertebral arch, with pedicles and laminae enclosing the vertebral foramen and giving rise posteriorly to the spinous process. Laterally, the transverse processes and superior articular processes frame the costal facet region, while the intervertebral discs above and below form smooth, pale intervals between adjacent vertebral endplates. Inferiorly, the stacked lumbar vertebrae continue toward the sacrum, shown as a blue triangular base. T12 matters because it blends thoracic and lumbar morphology, a practical problem when you need to count levels for surgery, injections, or reporting imaging. The inferior articular facets tend to assume a more lumbar-like orientation, and the twelfth rib articulates here, so a short or absent rib, a lumbosacral transitional vertebra, or hypoplastic transverse process can mislead level identification on radiographs or CT. Pedicle screws and posterior decompression at this junction demand respect for changing facet planes and the narrower canal compared with lower lumbar levels. A frequent pain generator.
