Description
Running from T1 to T12, the thoracic spine is presented as the midportion of the vertebral column between the cervical and lumbar regions, with each thoracic vertebra distinguished by costal demifacets on the posterolateral margins of the vertebral bodies for rib head articulation. Superiorly it transitions from C7 at the cervicothoracic junction, inferiorly it meets L1 at the thoracolumbar junction, and the thoracic kyphosis is apparent in lateral profile. Posterior elements, including the spinous processes that angle inferiorly in the mid thoracic levels, define the posterior contour of this segment. Segmental anatomy is clear. Emphasis on the thoracic vertebrae and their rib-bearing facets matters because this region behaves differently than the cervical and lumbar spine, motion is restrained by the rib cage and the orientation of the zygapophysial joints, while axial load transfer concentrates at the thoracolumbar junction. Clinically, the demifacet and full facet pattern helps explain costovertebral joint pain and the distribution of traumatic compression fractures, which often cluster around T7 to T9 and again at T12 in flexion injuries. The same landmarks guide level counting before thoracic epidural placement and posterior instrumentation, where misidentifying T12 or L1 changes the surgical plan. Use this illustration in gross anatomy and musculoskeletal courses to teach regional vertebral differences, thoracic kyphosis, and rib articulation points without the distraction of intervertebral disc detail.
