Description
Centered on the lumbosacral spine and pelvis in an anterior, radiograph-style view, the spinal cord vasculature is rendered as transparent vessels against the sacrum, coccyx, and paired hip bones (ilium, ischium, pubis) with the femoral heads seated laterally in the acetabula. A single anterior spinal artery courses inferiorly along the anterior median fissure at the midline, with smaller segmental contributions approaching it from a lateral direction as radicular branches enter near the intervertebral foramina. Fine arterioles and accompanying venous channels track longitudinally on the cord surface and communicate with the pial plexus, while adjacent neural elements in the pelvic region provide spatial context for the cord and its exiting roots. Midline versus paramedian anatomy is easy to read here. Seeing the dominant anterior spinal artery in relation to segmental inflow matters because anterior spinal artery syndrome is a pattern-recognition diagnosis: infarction classically affects the anterior two thirds of the cord, producing motor weakness and loss of pain and temperature with relative preservation of dorsal column modalities.
