Description
Cut through in longitudinal section, the adult testis is presented as a dense ovoid organ invested by the tunica vaginalis and a thick tunica albuginea, with fibrous septa radiating inward to partition lobules. Converging centrally, the septa form the mediastinum testis along the posterior border, where the rete testis sits before efferent ductules pass superiorly toward the head of the epididymis. Seminiferous tubules occupy the peripheral lobules, while the posterior aspect concentrates the excurrent pathways. Orientation matters. This cut section is the teaching view for correlating sperm production with sperm transport, and it clarifies why many intratesticular lesions are described relative to the tunica albuginea, mediastinum, and epididymal head on ultrasound. Clinically, the relationship between the testis and its coverings helps distinguish hydrocele (fluid within the tunica vaginalis) from hematocele and pyocele, while the posterior mediastinum and rete testis explain patterns of obstruction after epididymitis or post-vasectomy congestion. It also frames testicular torsion: twisting of the spermatic cord compromises venous return first, with downstream ischemia affecting the seminiferous epithelium.
