Description
Posterior elements of the lumbar vertebrae are presented in vertical alignment, with the spinous processes projecting posteriorly from the junction of the laminae and sitting in the midline between the paired superior and inferior articular processes. Intervertebral discs lie between adjacent vertebral bodies, their pale blue coloration separating them from the white cortical margins and trabecular texture of bone. A focal break is highlighted across one spinous process, with the fracture line traversing the distal portion of the bony projection and implying sharp, displaced fragments at its tip. Surrounding landmarks include the laminae forming the roof of the vertebral canal and the transverse processes extending laterally. Spinous process fractures in the lumbar spine often reflect traction or direct impact to the posterior column, with the supraspinous and interspinous ligaments and the paraspinal muscle attachments (multifidus and erector spinae) acting as deforming forces that can distract the fracture fragment. Pain is typically localized to the midline posterior lumbar region and worsens with extension or resisted trunk rotation. Although many isolated spinous process injuries are stable, this view helps you teach why posterior element fractures warrant assessment for associated injuries, such as pars interarticularis defects, facet complex trauma, or concomitant compression fracture of the vertebral body, depending on mechanism and energy.
