Description
Lumbar vertebrae are stacked in series with intervertebral discs separating adjacent vertebral bodies, and the illustrated fracture line traverses the cortical shell of one vertebral body near its superior endplate. Posterior to the damaged body, the pedicles converge toward the vertebral arch, maintaining the outline of the vertebral foramen and the start of the spinal canal. Superior and inferior endplates frame the nucleus pulposus and annulus fibrosus of the discs, while the posterior elements remain positioned dorsally relative to the injured anterior column. Compression fractures most often involve the anterior vertebral body and can create a wedge deformity that increases local kyphosis, a pattern commonly seen after a fall in an older male with reduced bone density or after axial loading trauma in a younger adult. Canal compromise is the teaching point: a benign-appearing crack in the body differs from a burst fracture, where retropulsed fragments encroach posteriorly toward the vertebral foramen and threaten the cauda equina. The preserved visibility of the foramen in this composition helps clarify the relationship between anterior column failure and the neural compartment, which guides decisions about bracing, vertebroplasty or kyphoplasty, and when posterior stabilization is considered.
