Description
Lateral male head and neck anatomy is rendered with the facial musculature removed to expose the buccal lymphatics along the cheek. Fine collecting lymphatic vessels course superficial to the maxilla and zygomatic bone, tracking anterior to the external ear and draining posteriorly and inferiorly toward the parotid (preauricular) nodes and the upper deep cervical chain. The mandibular and maxillary dentition, nasal cavity and paranasal sinuses, and the contour of the temporomandibular region provide bony orientation for these vessels. Small nodes sit in the buccal fat pad territory, lateral to the oral vestibule and superior to the angle of the mandible. Buccal lymphatic drainage is a common blind spot in head and neck teaching because it crosses the usual superficial versus deep facial planes and can bypass the submandibular basin. That matters when staging oral cavity and maxillary gingival malignancies, where metastases may first appear in buccinator-associated channels, parotid nodes, or upper jugular nodes rather than where you expect on a simple schematic. It also helps explain patterns of spread from odontogenic infection and midface cellulitis, and why swelling near the anterior border of the masseter can reflect nodal disease rather than salivary pathology.
