Description
Running across the lateral cheek, the buccal lymphatics appear as collecting vessels coursing superficial to the buccinator and adjacent to the orbicularis oris at the oral commissure, with the masseter and temporalis forming key lateral and superior landmarks. Superiorly, the network relates to the infraorbital region beneath orbicularis oculi, while inferiorly it trends toward the mandibular border and the upper cervical region where the sternocleidomastoid frames the neck. Blue facial and cervical veins, including the facial vein along the nasolabial and mandibular course and tributaries toward the external jugular system, provide positional reference for the lymphatic drainage pathways. Muscular layers for facial expression, including frontalis and zygomatic region musculature, set the depth relationships around the parotid and buccal spaces. Clinically, buccal lymphatic drainage explains early nodal spread from lesions of the cheek, upper lip, and adjacent gingivobuccal sulcus, with first-echelon drainage often reaching buccal nodes and then submandibular and upper deep cervical nodes. That matters when planning excision margins, sentinel lymph node mapping, or selective neck dissection levels I to II in oral cavity and cutaneous head and neck oncology. Close proximity to the facial vein and parotid region also clarifies why inflammation or malignancy in the buccal space can track along predictable routes despite variable superficial venous patterns. Small structures, big consequences.
