Description

Centered on the female shoulder girdle in anterior anatomical position, the deltoid is emphasized over the lateral aspect of the glenohumeral joint, wrapping from the clavicle and acromion toward its distal insertion on the deltoid tuberosity of the humerus. Anterior (clavicular) fibers lie superior and anterolateral to the pectoralis major and run inferolaterally, while the acromial portion forms the rounded contour of the shoulder lateral to the acromioclavicular region. Deep landmarks implied by the surface form include the humeral head and greater tubercle beneath the deltoid and the bicipital (intertubercular) groove medial to the anterior deltoid and lateral to the pectoralis major tendon. Breast tissue is rendered with a glandular and adipose pattern over the anterior thoracic wall, providing sex-specific context for surface relationships. Anterior deltoid anatomy matters when teaching the mechanics and examination of shoulder abduction and flexion, because the muscle’s broad origin on the clavicle and acromion explains its leverage and its tendency to mask deeper pathology on inspection. Injection technique depends on it. Intramuscular deltoid injections should land in the mid-deltoid, typically 2 to 3 fingerbreadths inferior to the acromion, avoiding the surgical neck region where the axillary nerve and posterior circumflex humeral artery traverse the quadrangular space; injury here classically produces deltoid weakness and a sensory deficit over the superior lateral arm.
