Description
Cut through in cross section, the testis is enclosed by the tunica albuginea, with tightly packed seminiferous tubules filling the parenchyma in convoluted profiles. Along the mediastinum testis, the seminiferous tubules drain into straight tubules (tubuli recti) and then the rete testis, drawn as a branching conduit that sits posteriorly and slightly superiorly relative to the bulk of the lobules. From that rete, multiple efferent ductules emerge as tapered, coiled tubules that course superiorly and posterolaterally to enter the head of the epididymis, which then continues as the more uniform ductus epididymidis along the posterior border of the testis. Vessels and cord elements lie lateral and superior, consistent with the spermatic cord’s approach to the superior pole. Efferent ductules are where sperm leaving the testis first encounter intense fluid resorption, and their transition from the rete testis to the epididymal head is a common stumbling block in teaching male reproductive tract continuity. Pathology often declares itself here. Obstruction or dysgenesis at the efferent ductules can present as epididymal cysts or contribute to obstructive azoospermia, and in vasectomy reversal planning the surgeon must distinguish epididymal obstruction from more proximal outflow issues. Their coiled, delicate course also explains why inflammatory epididymo-orchitis can disrupt sperm transport even when the seminiferous epithelium remains intact.
