Description
Encircling the adult male globe within the orbit, the four rectus muscles arise posteriorly and course anteriorly to insert on the sclera, with the medial rectus positioned nasally (medial) and the lateral rectus temporally (lateral), while the superior and inferior rectus lie along the superior and inferior aspects of the eyeball. Superior oblique runs from the superomedial orbit toward its trochlear pulley and then redirects posterolaterally to its scleral insertion, whereas inferior oblique originates anteromedially on the orbital floor and sweeps inferolaterally beneath the globe. Branches of the oculomotor nerve (CN III) track to the superior, inferior, and medial rectus and to inferior oblique, the trochlear nerve (CN IV) supplies superior oblique, and the abducens nerve (CN VI) reaches lateral rectus, a relationship best appreciated when you follow each nerve from the orbital apex toward its target. Anteriorly, the cornea overlies the iris and pupil, and fine episcleral vessels cross the sclera. Extraocular muscle anatomy is where ocular motility testing becomes anatomically explicit. Isolated weakness of lateral rectus from CN VI palsy produces impaired abduction and horizontal diplopia that worsens on gaze toward the affected side, while CN IV palsy weakens superior oblique and classically causes vertical diplopia, often worse on reading or stair descent, with compensatory head tilt. Inferior oblique overaction or restriction of inferior rectus in thyroid eye disease also reads directly off these lines of pull. Small structures, big consequences.
