Profile-oriented orbital anatomy frames the globe with the superior, inferior, medial, and lateral rectus muscles inserting onto the anterior sclera, while the superior and inferior oblique muscles course posteriorly to redirect force for torsional control. Medially and deep in the orbit, the medial and lateral recti can be traced back toward the common tendinous ring (annulus of Zinn) at the orbital apex, with the optic nerve (cranial nerve II) exiting the posterior pole of the eyeball. Anterior landmarks of the globe, including cornea, iris, and pupil, remain visible for orientation against the extraocular muscle insertions. Extraocular muscle relationships matter most at the apex, where the oculomotor nerve (cranial nerve III), abducens nerve (cranial nerve VI), and the ophthalmic division of the trigeminal nerve (V1) traverse the annular region and superior orbital fissure alongside the tendinous origin of the recti. This is the anatomy behind painful ophthalmoplegia in cavernous sinus pathology and the characteristic motility deficits of an isolated abducens palsy, where lateral rectus weakness leaves the eye deviated medially.
