Centered on the globe, the extraocular muscular cone encircles the sclera with the superior, inferior, medial, and lateral rectus muscles running from the posterior orbit toward their anterior insertions. Superiorly, the superior oblique courses forward toward the trochlear region at the superomedial orbital rim before turning posteriorly to pass deep to the superior rectus, while the inferior oblique arises anteromedially on the orbital floor and sweeps posterolaterally beneath the inferior rectus. Posterior relations are oriented toward the orbital apex where these muscles converge around the expected position of the optic nerve and the annulus of Zinn, defining the classic “muscular sleeve” that stabilizes the adult male eye during rotation. Precise depiction of these lines of pull matters when you are teaching ocular motility and localizing cranial neuropathies. The oculomotor nerve supplies the superior, inferior, and medial rectus and the inferior oblique, the trochlear nerve supplies the superior oblique, and the abducens nerve supplies the lateral rectus, a pattern that maps cleanly to exam findings such as ptosis with impaired adduction/elevation in CN III palsy, vertical diplopia worse on downgaze in CN IV palsy, or failure of abduction in CN VI palsy. Strabismus surgery also depends on this geometry, since recessions and resections act along each muscle’s arc of contact with the globe and can shift torsion when the obliques are involved.
