Description
Inferior cerebellar veins course over the inferior surface of the cerebellar hemispheres and vermis, tracking within the cerebellar fissures and along the tonsillar and biventral lobules before converging toward the midline venous collectors. Along the cerebellomedullary cistern, these venous channels lie posterior to the medulla oblongata and inferior to the tentorial surface, with short bridging veins approaching the adjacent dural sinuses. Blue venous pathways are contrasted against red arterial vessels and yellow cranial nerve rootlets around the brainstem, placing venous drainage in the same anatomic corridor as the vertebrobasilar circulation. Venous drainage on the underside of the cerebellum matters because it intersects the surgical and endovascular geography of the posterior fossa. Obstruction or thrombosis of inferior cerebellar venous outflow can produce cerebellar edema and hemorrhagic venous infarction that mimics arterial PICA territory stroke, a distinction that changes both imaging interpretation and acute management. In the operative setting, bridging inferior cerebellar veins can limit exposure during a midline suboccipital approach or telovelar route to the fourth ventricle, and inadvertent sacrifice risks postoperative swelling. Small vessels, big consequences.
