Description
Profiled in a lateral sagittal thoracic section, the heart sits anterior to the vertebral column with the great vessels rising superiorly, while a single lung appears posterolateral to the mediastinum. Coronary veins course over the epicardial surface in blue, converging toward the posterior atrioventricular (coronary) sulcus where the coronary sinus would be expected to run transverse and drain into the right atrium. Posteriorly, the azygos vein tracks superiorly along the right side of the thoracic vertebral bodies, then arches anteriorly above the root of the lung to join the superior vena cava. The trachea descends centrally and bifurcates toward the bronchial tree, with the rib cage and thoracic spine framing the cavity. A lateral section like this makes the venous relationships easier to teach than an anterior heart view because the posterior cardiac surface and the azygos arch share the same depth plane near the right hilum. That matters in practice. Central venous access, mediastinal surgery, and right-sided thoracic procedures all risk confusion between the azygos vein, superior vena cava, and adjacent pericardial reflections, and venous injury here can produce rapid hemothorax. Clear depiction of the coronary venous drainage also supports procedural anatomy for coronary sinus cannulation during cardiac resynchronization therapy and electrophysiology studies.
