Description
Anteriorly, the cardiac silhouette is centered in the thorax with the coronary venous network spread over the atria and ventricles. The great cardiac vein tracks superiorly in the anterior interventricular sulcus alongside the left anterior descending artery, then turns leftward in the atrioventricular (coronary) sulcus toward the coronary sinus. Smaller anterior cardiac veins course on the right ventricle and drain directly into the right atrium, while venules on the left ventricle converge toward the great cardiac vein near the base, inferior to the pulmonary trunk and ascending aorta. Orientation from the front matters because it matches what you see after median sternotomy, pericardiotomy, and anterior retraction of the lungs. Coronary venous anatomy becomes clinically concrete during cardiac resynchronization therapy and other transvenous procedures, where a lead is advanced via the coronary sinus into a lateral or posterolateral tributary to pace the left ventricle; an anterior view helps explain why tributary angulation, small-caliber venules, or a prominent Thebesian valve can complicate cannulation.
