Description
Posteriorly oriented torso anatomy is overlaid on the back of an obese adult Black male, allowing the thoracic and abdominopelvic viscera to be read against external landmarks. Superiorly, the trachea descends in the midline to the carina and main bronchi, with the right and left lungs occupying the posterolateral thorax; the heart sits slightly left of midline, mostly anterior to the vertebral column and partly overlapped by the left lung silhouette. Inferior to the diaphragm, the liver fills the right upper abdomen (appearing on the left side of the image), while the stomach lies more to the left, with the pancreas tucked posterior to it; paired kidneys sit retroperitoneally on either side of the spine, and the small intestine coils centrally with the large intestine framing it more peripherally. Obesity changes how clinicians think about “space” inside the trunk, and this posterior composite makes that point plainly. Hepatomegaly and fatty infiltration can expand the liver’s cranio-caudal footprint, crowding the gastric region and limiting diaphragmatic excursion, which correlates with reduced functional residual capacity and perioperative atelectasis risk. The kidney position relative to the 12th rib and paraspinal musculature also matters when planning posterior approaches for percutaneous nephrolithotomy or when interpreting flank pain patterns. Anatomy faculty can drop this into units on thoracoabdominal topography, surface anatomy, or body habitus effects on physical examination, pairing it with palpation landmarks and percussion boundaries.
