Centered within the thoracic cavity, the heart sits in the middle mediastinum between the right and left lungs, with the diaphragmatic domes forming the inferior boundary of the pleural spaces. Inferior to the diaphragm, the stomach occupies the left upper quadrant, while the pancreas lies posterior to it in the upper retroperitoneal region and the small intestine fills the central abdomen in layered loops. Framing the small bowel, the large intestine courses peripherally, and the kidneys appear posterolateral to the vertebral column, partly obscured by overlying viscera. Tight packing. Age and body habitus change how you read gross anatomy, and an elderly male specimen often highlights clinically familiar relationships: an elongated thorax, a relatively low diaphragmatic position, and a more capacious abdominal cavity that alters how the bowel drapes over the retroperitoneum. That matters when teaching why pleural effusions track into the costodiaphragmatic recess, why basal atelectasis can mimic abdominal pain, and how posterior organs like the kidneys and pancreas remain relatively fixed while mobile intraperitoneal bowel shifts with distension. Even without vascular detail, the organ-to-organ adjacency supports discussions of referred pain patterns and approach corridors for open and laparoscopic access.
