Running longitudinally on either side of the spinous processes, the longissimus thoracis forms the intermediate column of the erector spinae, positioned lateral to spinalis and medial to iliocostalis. A posterior view of the male back would place its broad tendinous origin inferiorly along the sacrum, posterior iliac crest, and lumbar spinous processes via the thoracolumbar fascia, then track superiorly across the lumbar transverse processes toward the thoracic transverse processes and adjacent rib angles. The vertebral column and posterior ribs provide the deep bony scaffold, while the scapulae sit posterolaterally over the upper thorax, helping orient the muscle’s lateral border near the costovertebral region.
