Description

Anterior thoracic anatomy is presented on a standing Asian male with the trachea descending in the midline of the neck into the thorax, where it bifurcates at the carina into the right and left main bronchi. Each bronchus enters the lung at the hilum on the medial surface, with the right lung positioned slightly more superiorly and appearing broader, while the left lung sits more inferolaterally to accommodate the cardiac impression. Interlobar fissures divide the lobes: an oblique fissure on both sides and a horizontal fissure on the right, separating superior, middle, and inferior lobes on the right and superior and inferior lobes on the left. Lobar organization is clear. This front-facing presentation matters when teaching surface anatomy and airway branching as it relates to aspiration and bronchoscopy. The more vertical, wider right main bronchus explains why aspirated foreign bodies often lodge in the right bronchial tree, commonly the bronchus intermedius or right lower lobe bronchi, while the left side’s longer, more oblique course changes endoscopic angles and tube positioning. It also orients learners to the clinical concept of lobar localization, tying fissure anatomy to lobar pneumonia patterns and to where pleural fluid layers dependently.
