Posterior cervical soft tissues are rendered semi-translucent to reveal semispinalis cervicis within the transversospinalis group, lying deep to trapezius and the splenius muscles in the upper back and neck of an adult male. Its vertically oriented fascicles course superomedially from transverse processes of the upper thoracic vertebrae toward the spinous processes of C2 to C5, immediately lateral to the midline spinous processes and the nuchal ligament. Deeper and more medial, multifidus occupies the laminar gutter, while semispinalis capitis continues superiorly toward the occiput; laterally, levator scapulae ascends from the superior angle of the scapula to the transverse processes of the upper cervical spine. Skin and superficial fascia remain partially intact. Semispinalis cervicis matters because it is a primary segmental extensor and rotator of the cervical spine, and its deep position makes it a frequent point of confusion when teaching layered posterior neck anatomy or when interpreting posterior cervical muscle edema after trauma. Surgeons working through a posterior cervical approach must split or reflect paraspinal muscle planes to reach the laminae and facet joints, and muscle stripping at C2 to C5 can contribute to postoperative axial neck pain and loss of extension endurance. In cervical whiplash and chronic neck pain, selective dysfunction of the deep extensors, including semispinalis and multifidus, is a practical rehabilitation target.
