Description
Running superficially along the anterior cervical region, the sternohyoid is presented as a vertical infrahyoid strap muscle extending from the posterior aspect of the manubrium sterni and adjacent medial clavicle superiorly to the inferior border of the corpus ossis hyoidei. In lateral profile, it sits anterior to the laryngeal framework and upper tracheal rings, with the thyroid cartilage and cricoid cartilage forming firm midline landmarks just posterior to its course. Posterolateral to the strap plane, the sternocleidomastoid sweeps obliquely from the sternum and clavicle toward the mastoid process, creating a clear contrast between the superficial lateral neck contour and the flatter midline infrahyoid layer. Simple geometry, but clinically loaded. Infrahyoid anatomy matters when you teach or operate in the anterior neck because these muscles define the first layer encountered after skin and platysma in midline approaches. During tracheostomy or emergency cricothyrotomy, the sternohyoid is commonly separated along the linea alba cervicalis and retracted laterally to expose the pretracheal fascia and trachea, and this illustration helps learners understand why staying in the midline minimizes bleeding and avoids the carotid sheath contents. The relationship to the larynx also supports functional teaching: sternohyoid depresses the hyoid after swallowing, working with the other strap muscles (sternothyroid, thyrohyoid, omohyoid) to reset the suprahyoid complex.
