Description
Running vertically in the anterior neck, the sternohyoid muscle is shown as a paired infrahyoid strap muscle lying immediately deep to the superficial cervical fascia and superficial to the laryngeal framework. Each belly ascends from the posterior aspect of the manubrium sterni and adjacent medial clavicle, then continues superiorly toward its insertion on the inferior border of the hyoid bone, remaining close to the midline. Lateral to the sternohyoid, the sternocleidomastoid forms a prominent superficial boundary from the mastoid process to the sternum and clavicle, while the mandibular outline and cervical vertebrae provide bony orientation for the suprahyoid and prevertebral compartments. Clinically, this anterior, under-the-skin presentation matches the first layers encountered in a midline cervical approach, where surgeons split or retract the strap muscles to reach the thyroid gland, trachea, or upper mediastinum. Clear appreciation of the sternohyoid’s fascial investment matters when teaching the pretracheal fascia and the way infection or hematoma can track in the anterior neck. Keep the hyoid in mind, the sternohyoid’s superior attachment makes it a practical landmark for correlating swallowing mechanics and laryngeal elevation, even though the muscle itself depresses the hyoid after a swallow.
