Description
Beneath a translucent cutaneous outline of the male face, the levator labii superioris alaeque nasi is positioned anterolateral to the nasal cartilages and medial to the levator labii superioris proper, running inferiorly from the frontal process of the maxilla toward the upper lip and the alar margin. Deep to it, the maxilla forms the bony substrate of the lateral nasal wall and anterior maxillary region, while the nasal bones and septal and alar cartilages define the midline framework superiorly. The mandible and dentition sit inferior to the oral aperture, with the hyoid suspended in the suprahyoid region and the cervical vertebrae aligned posteriorly. Small footprint, strong effect. This specific muscle is a key driver of upper lip elevation and alar flare, the combined movement that contributes to expressions such as sneering and to functional widening of the nostril during forced inspiration. Clinically, its course over the anterior maxilla makes it relevant in cleft lip repair planning and in rhinoplasty when surgeons must preserve or intentionally release perinasal muscular attachments that influence alar base position and dynamic nasal valve behavior. It also explains why perinasal injections of botulinum toxin can soften “bunny lines” and reduce excessive alar elevation, but risk subtle asymmetry of the upper lip if dosing drifts laterally.
