Habenulae occupy the dorsomedial aspect of the epithalamus, perched just superior to the thalamus and bordering the roof of the third ventricle in a dorsal view of the human diencephalon and adjacent brainstem. Each habenula appears as a small rounded eminence lateral to the pineal region, with the habenular commissure spanning the midline posterior to the third ventricle and anterior to the pineal gland. Medially, the stria medullaris thalami courses toward the habenular nuclei along the superior thalamic surface, while posteriorly the pineal stalk and nearby posterior commissure sit at the junction of diencephalon and midbrain tectum. Small structure, big consequences. Habenular circuitry links limbic forebrain input to midbrain monoaminergic centers, so these nuclei routinely enter discussions of reward prediction, aversion processing, and mood regulation. Lesions or mass effect in the pineal region (pineal cysts, germinomas, tectal gliomas) can distort the posterior third ventricular roof and compress the habenular commissure, an anatomic relationship worth showing explicitly when teaching dorsal diencephalic anatomy and correlating with Parinaud syndrome or obstructive hydrocephalus patterns. The fixed dorsal perspective makes the epithalamus easier to orient than in coronal sections, where habenular contours can be lost against the thalamic pulvinar and adjacent commissures. Neuroanatomy faculty can drop this illustration into epithalamus modules to anchor the spatial relationships among thalamus, pineal gland, and the dorsal midbrain, and to label the stria medullaris and commissural landmarks in a single frame.
