Positioned on the posterolateral aspect of the elbow, the anconeus is highlighted as a small triangular muscle spanning from the lateral epicondyle of the humerus to the lateral olecranon and proximal posterior surface of the ulna. Its fibers run inferomedially, lying just distal to the posterolateral margin of the elbow joint line and adjacent to the lateral surface of the joint capsule. More distally, the posterior compartment of the forearm continues toward the wrist with extensor muscle bellies transitioning into long tendons that course over the dorsum of the hand toward the digits.
