Description
Positioned on the posterolateral aspect of the elbow, the anconeus is highlighted as a small, triangular muscle spanning from the posterior surface of the lateral epicondyle of the humerus to the lateral margin of the olecranon and proximal posterior ulna. Deep to it lie the distal humerus, the olecranon process, and the proximal radioulnar region, with the radius lateral to the ulna and the humeroulnar joint centered medially. Its fibers run inferomedially across the elbow capsule, sitting just distal and posterior to the common extensor origin at the lateral epicondyle. Clear bony landmarks anchor the orientation. Clinically, this posterior view matters because the anconeus forms a reliable surface landmark for the posterolateral approach to the elbow, helping orient dissection and exposure around the lateral epicondyle, olecranon, and proximal ulna without losing the joint line. Pain at the lateral elbow is often attributed to lateral epicondylitis, but focal tenderness and swelling at the posterolateral gutter can implicate the anconeus or the adjacent capsule, and the muscle is sometimes hypertrophied in athletes who repetitively extend the elbow. It also reinforces a teaching point that the anconeus assists triceps brachii in elbow extension while subtly stabilizing the elbow and influencing ulna position during forearm rotation. Small muscle, big landmark.
