Description
Seen from a lateral perspective at the elbow, the anconeus appears as a small triangular muscle spanning the posterolateral aspect of the joint, extending from the lateral epicondyle of the humerus to the lateral margin of the olecranon and proximal posterior ulna. Its fibers run obliquely inferiorly and medially, lying just posterior to the radiocapitellar region and immediately lateral to the triceps brachii tendon as it inserts on the olecranon. Bony landmarks you would expect to read clearly in this view include the lateral epicondyle, capitulum, radial head, and the subcutaneous olecranon. Compact anatomy. Clinically, this muscle matters because it overlies the posterolateral capsule and forms part of the working corridor for lateral and posterolateral elbow exposures, including approaches that navigate between anconeus and extensor carpi ulnaris. Its relationship to the lateral ulnar collateral ligament complex makes it relevant when teaching posterolateral rotatory instability and when orienting repairs after elbow dislocation. Pain at the lateral elbow is not always extensor tendinopathy, anconeus strain and adjacent capsular irritation can mimic lateral epicondylitis in throwing athletes and manual workers.
