Description
Seen from a lateral perspective, the anconeus is isolated along the posterolateral elbow, arising from the lateral epicondyle of the humerus and coursing distally to the lateral aspect of the olecranon and adjacent proximal ulna. Its fibers sit immediately posterior to the common extensor origin and anteromedial to the superficial contour of the triceps brachii tendon as it approaches the olecranon. Superficial landmarks include the lateral epicondyle superiorly and the proximal ulna inferiorly, with the radial head and capitulum expected just anterior to the muscle belly in this projection. Small muscle, tight space. This view matters because the anconeus marks the posterolateral corner of the elbow where pain generators and surgical intervals cluster, and it is frequently discussed during assessment of lateral elbow pain when lateral epicondylitis, radiocapitellar chondropathy, or posterolateral rotatory instability enter the differential. Its close attachment to the capsule and proximal ulna makes it a useful reference when teaching how elbow extension couples with subtle stabilization of the radioulnar complex, and why edema or scarring in the posterolateral gutter can limit terminal extension. For surgeons, the anconeus also orients the posterolateral approach to the proximal ulna and lateral elbow, where protecting the nearby posterior interosseous nerve in the supinator is a recurring concern.
