Description
From a lateral perspective of the male elbow, the anconeus appears as a small triangular muscle on the posterolateral aspect of the joint, spanning from the lateral epicondyle of the humerus to the proximal ulna along the lateral margin of the olecranon and adjacent posterior surface. Its fibers run inferomedially, lying posterior to the common extensor origin and immediately superficial to the posterolateral joint capsule. Medially, the olecranon and subcutaneous border of the ulna form a bony reference, while the anconeus sits just distal and posterior to the lateral epicondyle. Although easy to overlook, the anconeus matters when you are teaching elbow stability and posterolateral anatomy. It acts with the triceps brachii to assist elbow extension and is often described as a dynamic tensor of the elbow capsule, helping to limit capsular impingement during extension. Lateral elbow pain workups and surgical approaches to the radiocapitellar region benefit from clear orientation to this muscle, since interval planning and protection of nearby structures, including the course of the radial nerve in the lateral elbow region, depend on accurate surface and deep landmarks. Small muscle, real consequences.
