Description
Positioned on the posterolateral aspect of the elbow, the anconeus is highlighted beneath the skin as a small triangular muscle spanning from the lateral epicondyle of the humerus to the proximal ulna, blending along the lateral margin of the olecranon. Proximal to it, the lateral head of triceps brachii descends toward the common triceps tendon inserting on the olecranon, while the brachioradialis and extensor carpi radialis longus occupy a more anterior and lateral plane along the supracondylar region. Distally, the extensor compartment of the forearm continues toward the dorsum of the hand, with long extensor tendons tracking across the wrist into the digits. The anconeus sits superficial to the elbow capsule and posterolateral ulnohumeral joint. This lateral perspective matters when teaching the posterolateral corner of the elbow and the transition between triceps and the extensor-supinator mass, because the anconeus is a reliable surface landmark for the interval used in the Kocher approach (between anconeus and extensor carpi ulnaris) to access the radial head and capitellum. In trauma and sports cases, orienting the anconeus to the lateral epicondyle and olecranon helps clarify where posterolateral rotatory instability can originate after injury to the lateral ulnar collateral ligament complex, and why surgical dissection respects the posterior interosseous nerve’s course as exposure moves anteriorly.
