Description
Anteriorly, the annular ligament (ligamentum anulare radii) forms a dense ring around the radial head, spanning from the anterior margin of the radial notch of the ulna to its posterior margin and completing the proximal radioulnar joint. The radial head sits lateral to the ulnar trochlear notch, while the radial neck continues distally beneath the ligament as it transitions toward the interosseous membrane of the forearm. Expect the capsule of the elbow joint to blend with the annular fibers near the proximal radioulnar recess, with the radial collateral ligament complex lying laterally in close relationship. This anterior perspective matters because it frames the annular ligament as the primary soft-tissue restraint maintaining radial head apposition during pronation and supination, while still permitting rotation against the ulna. In pediatric radial head subluxation (nursemaid’s elbow), traction on the pronated, extended forearm can allow the radial head to slip partially from the annular ring, making the contour and attachment sites of the ligament the key teaching point during reduction maneuvers.
