Description

Arising from the ileocolic artery, the anterior cecal artery courses on the anterior surface of the cecum, running inferior and slightly lateral toward the haustrated wall before breaking into small mural branches. The cecum sits inferior to the terminal ileum and medial to the ascending colon, with epiploic appendices draping the serosa as pale lobules. Paired cecal veins accompany the arterial supply in close apposition, and small ileocolic lymph nodes cluster along the mesenteric border near the vascular pedicle. Fine autonomic nerve filaments track with the vessels. Anterior cecal arterial anatomy matters when the cecum is mobilized, inflamed, or resected, because the anterior and posterior cecal arteries are short, variable branches that can be avulsed during traction on the ileocecal region. Bleeding here is often brisk. This anterior perspective also clarifies why some cases of cecal ischemia or ulceration localize to the anterior wall, where end-arterial territories and tension from cecal distension can limit collateral flow from adjacent colic branches.
