Arcuate artery anatomy is presented from a dorsal (superior) view of the male foot, with the vessel arching transversely across the proximal metatarsals just distal to the tarsometatarsal joints. From its lateral origin near the dorsalis pedis artery, the arcuate artery courses medially and gives off dorsal metatarsal arteries that run distally in the intermetatarsal spaces, then continue as dorsal digital branches toward the toes. Dorsal venous channels accompany the arterial tree more superficially, while the metatarsal bases and dorsal midfoot provide the bony backdrop for the characteristic curve. Course and branching of the arcuate artery matter when you are correlating pedal pulses and planning dorsal approaches to the midfoot. Incisions for Lisfranc fracture-dislocation fixation, dorsal midfoot arthrodesis, or exposure of the tarsometatarsal joints can place the dorsal metatarsal arteries at risk, and an injured or vasospastic branch can compromise toe perfusion in patients with peripheral arterial disease or diabetes. Variants are common, including a diminutive arcuate artery with dorsal metatarsal supply arising directly from dorsalis pedis, so a clean dorsal map helps explain why Doppler signals and ischemic patterns do not always match textbook expectations.
