Description
Arcing across the dorsum of the foot, the arcuate artery courses laterally deep to the extensor tendons and over the bases of the metatarsals, forming a curved conduit between the dorsal pedis artery medially and the dorsal metatarsal arteries distally. From a lateral profile, the vessel sits anterior to the tarsal bones and metatarsals, with the fibula and lateral malleolus proximal and posterior, and the calcaneus positioned inferior and posterior in the hindfoot. Accompanying veins track in parallel, rendered in blue, while the tibia, fibula, talus, calcaneus, cuboid, and metatarsals provide a rigid landmark framework. It is a clean topographic map of dorsal foot vascular anatomy. Lateral visualization of the arcuate artery matters because the dorsal circulation is often used as a clinical window into peripheral arterial disease, and the dorsal pedis and its continuation into the arcuate system can be diminished in atherosclerosis or diabetes-related vasculopathy. Palpation of the dorsalis pedis pulse and interpretation of a weak or absent signal becomes more meaningful when you can trace expected branching toward the dorsal metatarsal arteries and their digital contributions, while remembering that anatomic variation and collateral flow through the plantar arch can mask proximal disease. The relationship to the metatarsal bases also explains why dorsal approaches for midfoot fixation or Lisfranc procedures demand careful dissection to avoid arterial injury.
