Description
Seen from a posterior perspective in an adult male, the large intestine is rendered from cecum and ascending colon on the anatomic right, across the transverse colon, to the descending and sigmoid colon on the left before continuing inferiorly as the rectum. Arterial supply tracks along the dorsal mesenteric border, with branches of the superior mesenteric artery (ileocolic, right colic, and middle colic arteries) distributing to the cecum, ascending colon, and proximal two thirds of the transverse colon, while the inferior mesenteric artery (left colic and sigmoid arteries) supplies the distal transverse colon, descending colon, and sigmoid. Along the colonic wall, the marginal artery of Drummond links these territories near the mesenteric attachment, with straight vasa recta, arterioles, approaching the bowel between haustra, adjacent to the taeniae coli and appendices epiploicae. Orientation is clear. Right stays right. A posterior vascular view matters because it mirrors how the mesocolon is handled in colectomy, where ligation occurs at the pedicle and dissection proceeds along the retroperitoneal plane before the bowel is mobilized. Watershed zones become easier to teach here, at Griffiths point near the splenic flexure and at Sudeck point near the rectosigmoid junction, both relevant to ischemic colitis and to anastomotic perfusion after left-sided resections. The collateral pathway between SMA and IMA territories is a practical concept, not an academic one.
