Description
Arising from the internal iliac system and the inferior mesenteric artery, the superior, middle, and inferior rectal arteries course along the lateral pelvic wall and converge on the rectal wall as they pass through the pelvic fascia. Proximally, the descending colon continues into the sigmoid colon, where haustra and the taeniae coli are suggested by the sacculated contour and faint longitudinal banding; distally, the sigmoid tapers toward the rectum. From this lateral perspective, epiploic (omental) appendages hang from the antimesenteric border, while arterial branches arc over the serosa and send penetrating twigs toward the bowel wall. Curved anastomoses between sigmoid branches and the superior rectal artery are implied along the rectosigmoid junction. This angle matters because it tracks the extramural approach of rectal vessels in the same corridor used during total mesorectal excision, where staying in the correct mesorectal plane limits bleeding while preserving autonomic nerves running in adjacent pelvic fascia. The superior rectal artery also marks a watershed territory near the rectosigmoid region, a common site for ischemic colitis when perfusion drops or when the inferior mesenteric artery has been ligated in colorectal surgery.
