Description
Seen from the side, the distal large bowel is rendered with the rectum continuing superiorly into the sigmoid colon, with haustra and the longitudinal teniae coli tracking along the colonic wall and epiploic (omental) appendices projecting from the serosal surface. Blue venous channels course within and along the rectal wall as the superior rectal vein ascends proximally toward the inferior mesenteric vein, while the middle and inferior rectal veins run more laterally and inferiorly toward the internal iliac system. Anastomotic connections are suggested in the submucosa, where portal drainage from the superior rectal territory meets systemic drainage from the middle and inferior rectal territories. Orientation is clear. Superior lies toward the descending colon, inferior toward the anal canal. Portal-systemic anastomoses at the rectal venous plexus matter because they are one of the classic sites of portosystemic collateralization in portal hypertension, producing internal hemorrhoids (painless rectal bleeding) as the superior rectal tributaries dilate within the submucosa. The lateral perspective also tracks the relationship of the middle rectal vessels to the pelvic sidewall and mesorectal envelope, a practical concern during total mesorectal excision, where bleeding control and nerve preservation depend on recognizing where venous branches cross toward the internal iliac region. Confusion with external hemorrhoids is common. This view helps prevent it.
