Posterior to the anal canal, the inferior rectal veins form a venous network within the ischioanal fossa and along the external anal sphincter, then course laterally to drain into the internal pudendal vein. Superiorly, the muscular wall of the rectum (muscularis externa) continues into the distal sigmoid colon, while the veins track in the perirectal connective tissue before converging toward the pudendal canal (Alcock canal) on the lateral pelvic wall. Yellow lobules and nodules represent perianal fat and regional lymphatic tissue embedded in the ischioanal fat. Venous anatomy dominates this field. For teaching anorectal venous return, a posterior view clarifies where external hemorrhoids arise, below the pectinate line, from dilated veins of the inferior rectal system rather than the superior rectal (portal) drainage. That distinction matters clinically when correlating perianal thrombosis with pain and somatic innervation via inferior rectal nerves traveling with the internal pudendal vessels. Surgeons working around the ischioanal fossa, whether for drainage of an ischioanal abscess or during fistula surgery, use these vessels as a practical warning for bleeding during lateral dissection.
