Rectal Veins

Description

Centered on the distal large intestine, the rectum is rendered with its mucosal folds and muscular wall, while an anastomosing network of rectal veins forms superficial and deep venous plexuses around the anorectal segment. Superiorly, veins track proximally toward the rectosigmoid junction in continuity with the superior rectal vein, while inferior and middle channels course more laterally within the pelvic tissues consistent with drainage to the internal iliac system. Submucosal veins lie closest to the rectal lumen, and a more external plexus invests the rectal wall, creating the layered appearance typical of anorectal venous anatomy. Rectal venous drainage is a classic portosystemic crossroads: the superior rectal vein drains to the inferior mesenteric vein and portal system, while the middle and inferior rectal veins drain to the internal iliac and internal pudendal veins, respectively. That junction matters when you are teaching or planning around hemorrhoidal disease, because internal hemorrhoids arise from dilation of the internal (submucosal) plexus above the pectinate line, whereas external hemorrhoids involve the external plexus below it. Portal hypertension can enlarge these anastomoses and produce anorectal varices, a distinct entity from routine hemorrhoids and a common point of confusion in clinical education.
