Description
Posteriorly oriented anatomy of the male rectum and distal large intestine is rendered with emphasis on the rectal venous plexuses. Tributaries around the rectal ampulla converge superiorly into the superior rectal vein along the mesorectum, continuing toward the inferior mesenteric system, while inferior channels track toward the middle and inferior rectal veins, communicating with the internal iliac and internal pudendal pathways. Haustrated sigmoid colon sits superior to the rectum, and appendices epiploicae punctuate the serosal surface laterally. Venous caliber and branching are kept readable. Rectal venous drainage matters because it bridges portal and systemic circulations at a predictable site, the anorectal junction, where portosystemic anastomoses enlarge in portal hypertension and present clinically as internal hemorrhoids. A posterior view clarifies the longitudinal course of the superior rectal vessels within the mesorectal envelope, a relationship that informs total mesorectal excision and helps explain patterns of bleeding during low anterior resection or proctectomy. For trainees, this is also the cleanest way to separate superior rectal (portal) from middle and inferior rectal (systemic) drainage without pelvic viscera obscuring the field.
