Description
Posterior to the male rectum, the middle rectal vein is traced as a blue venous channel approaching the rectal wall within the lateral ligament of the rectum, where it runs lateral to the muscularis and medial to the pelvic sidewall. Superiorly, the rectum continues toward the sigmoid colon, identifiable by haustra, while inferiorly the distal rectum tapers toward the anorectal junction. Small pericolic and pararectal lymph nodes are indicated along the adjacent bowel, reinforcing the relationship between venous and lymphatic pathways. Orientation stays firmly posterior, so the vessel’s course can be read against the back surface of the rectum rather than the peritoneal reflections. Surgeons care about this vein because it sits in the same operative corridor as the lateral stalks controlled during total mesorectal excision and low anterior resection, where brisk pelvic bleeding can follow an overly deep bite into the lateral ligament. It also helps explain portosystemic connections: the middle rectal vein drains to the internal iliac system, interfacing with the superior rectal (portal) territory and the inferior rectal veins, a setup that matters when teaching why anorectal varices differ from typical hemorrhoids in portal hypertension. Bleeding happens fast. Use this asset in pelvic anatomy and colorectal surgery teaching, in operative atlases illustrating rectal mobilization and ligation planes, or in manuscripts and slide decks discussing rectal cancer spread and pelvic venous drainage patterns in the adult male.
