Superior Rectal Vein

Description

Arising from the rectal venous plexus in the upper rectum, the superior rectal vein courses superiorly in the mesorectum and then continues within the sigmoid mesocolon alongside the sigmoid colon. Tributaries track along the taeniae coli and between haustra, converging toward the mesenteric border where the venous channels run medial to the colonic wall and deep to the serosa. Yellow nodal structures sit along these vessels in the mesentery, consistent with pararectal and sigmoid lymph nodes positioned near the vascular pedicles. Relationships are read best from distal rectum to proximal sigmoid. From a clinical standpoint, the superior rectal vein matters because it forms part of the portal venous drainage pathway, typically via the inferior mesenteric vein, and it defines the vascular axis surgeons respect during low anterior resection and total mesorectal excision. Portal hypertension can enlarge this system and contribute to internal hemorrhoids at the superior rectal venous plexus, a common point of confusion with external hemorrhoids drained by systemic tributaries. Lymphatics running with the superior rectal vessels guide oncologic staging, since upper rectal and sigmoid tumors often spread along these mesenteric chains before reaching more proximal nodes.
