Description
Viewed laterally, the atlantooccipital joint capsule spans between the occipital condyle of the basiocciput and the superior articular facet of the atlas (C1), forming the synovial envelope of the craniovertebral junction. The capsule sits immediately inferior to the occipital bone and superior to the posterior arch of C1, with the mastoid region and zygomatic arch lying anterior and lateral in the same field of view. Adjacent osseous landmarks such as the lambdoid suture, posterior cranial vault, mandibular body with dentition, and upper cervical spinous and transverse processes help anchor the capsule’s position in the upper neck. A clean lateral perspective matters because it aligns with how clinicians conceptualize flexion and extension at the atlantooccipital articulation and how the capsule restrains end-range motion.
