Description
Seen in lateral profile, the occipital bone of the skull sits superior to the atlas (C1), whose ring-like anterior and posterior arches cradle the craniovertebral junction rather than forming a typical vertebral body. Immediately inferior, the axis (C2) lies posterior to the pharyngeal airway shadow line and articulates with C1 at the atlanto-axial joints, while the remaining cervical vertebrae step down in a gentle lordotic curve. Cranial sutures (coronal, sagittal, lambdoid) are rendered along the calvaria, and the maxilla, mandible, and dentition establish the anterior facial framework. A clean side view makes the biomechanics of the upper cervical spine easy to teach: flexion and extension concentrate at the atlanto-occipital joints, while rotation is dominated by the C1-C2 complex around the dens of C2, even when the dens itself is partly obscured in strict lateral projection. This is the anatomy interrogated in trauma imaging when you suspect atlanto-occipital dissociation, Jefferson fracture of C1, or odontoid injury, and it is also the target region for preoperative planning around the craniovertebral junction. Alignment matters. Small offsets between the occiput, C1, and C2 can be diagnostic. Use this illustration in gross anatomy and radiographic anatomy courses when introducing cervical vertebrae, as well as in spine or neurosurgical teaching files that explain cervical instability, fracture patterns, or the rationale for C1-C2 fixation. It also suits medical publishing contexts that need a gender-specific skeletal reference for the skull and upper spine with C1 and C2 highlighted for rapid orientation.
